Summary

This study details a rare case of angiosarcoma spreading to the bone marrow in a patient with primary splenic angiosarcoma. Splenic angiosarcomas show a unique tendency for bone marrow infiltration.

Background:

  • Angiosarcomas are rare, aggressive vascular tumors.
  • Metastasis is common, but bone marrow involvement is infrequently documented.

Observation:

  • A 36-year-old man with high-grade splenic angiosarcoma developed leukoerythroblastic anemia 3 years post-splenectomy.
  • Bone marrow biopsy confirmed extensive angiosarcoma infiltration with characteristic spindle cells and vascular channels.

Findings:

  • Immunohistochemistry revealed tumor cells positive for endothelial markers CD31, CD34, and von Willebrand factor.
  • This is one of only three reported cases of angiosarcoma bone marrow metastasis, all originating from the spleen.

Implications:
