Internet and Telephone Support for Discontinuing Long-Term Antidepressants: The REDUCE Cluster Randomized Trial.

JAMA network open·2024

Personality disorder (PD) affects 22% of community adults with depressive disorders. While PD impacts long-term outcomes, initial social function is the sole independent predictor of recovery in depressive disorder patients.

Area of Science:

  • Psychiatry
  • Clinical Psychology
  • Epidemiology

Background:

  • Limited data exists on the prevalence of personality disorder (PD) in community samples of individuals with depressive disorder.
  • The impact of PD on service utilization and outcomes in these populations remains understudied.

Purpose of the Study:

  • To investigate the prevalence of PD in a community sample with depressive disorder.
  • To examine the influence of PD on treatment utilization and patient outcomes over a 1-year period.

Main Methods:

  • A two-stage screening study was conducted across five European countries to identify depressive disorder cases using SCAN.
  • Personality was assessed six months post-diagnosis, with follow-up assessments of symptoms and social function over one year.

Related Experiment Videos

Main Results:

  • Personality disorder was identified in 22% of the community sample with depressive disorders, with prevalence varying by country (13.7%-33.3%).
  • Cluster C personality disorders constituted 43% of cases. Long-term psychotropic drug use was higher in the PD group, even when controlling for depression severity.
  • While individuals with PD showed higher initial symptom scores and were more likely to be cases at follow-up, this association diminished when controlling for depression severity. Initial social function was the only independent predictor of 6- and 12-month outcomes.

Conclusions:

  • Personality disorder is prevalent in non-treatment-seeking individuals with depressive disorder.
  • PD influences outcomes in depressive disorder, but this is mediated by initial mood severity.
  • Social function is a critical, independent predictor of outcomes and warrants separate assessment in individuals with depressive disorder.
