Area of Science:

  • Gastroenterology
  • Surgical Pathology

Background:

  • Bezoars are concretions in the gastrointestinal tract, commonly in the stomach.
  • Impaired gastric emptying and reduced acid production post-gastric surgery are primary risk factors.
  • Phytobezoars (vegetable matter) are more prevalent than trichobezoars (hair).

Observation:

  • A retrospective review of hospitalized patients from 1992-2002 identified 18 bezoar cases.
  • The majority (16) were phytobezoars, with 2 cases of trichobezoars.
  • Eleven patients had a history of gastric surgery.

Findings:

  • Surgical intervention was the primary treatment for nearly all patients.
  • Two rare cases of trichobezoar were specifically documented.
  • Phytobezoars constituted the most common type encountered.

Implications:

  • Clinicians should maintain a high index of suspicion for bezoars in patients with a history of gastric surgery.
  • Early diagnosis through methods like endoscopy and gastrografin swallow is crucial.
  • Understanding risk factors aids in timely diagnosis and management of bezoar formation.
