Abnormal parathyroid hormone (PTH) secretion is common in advanced heart failure. PTH release is depressed during exercise stress in these patients, suggesting parathyroid gland dysfunction.
Area of Science:
- Cardiology
- Endocrinology
Background:
- Parathyroid hormone (PTH) regulates calcium and influences myocardial function.
- Abnormal PTH secretion is linked to impaired heart muscle performance.
Purpose of the Study:
- To investigate baseline PTH levels in advanced congestive heart failure.
- To assess PTH and calcium response to exercise stress in heart failure patients.
Main Methods:
- Measured baseline PTH in 27 advanced heart failure patients (LVEF: 17 +/- 9%).
- Conducted symptom-limited exercise stress tests on nine patients, monitoring PTH and blood calcium.
- Analyzed PTH and calcium changes during exercise and recovery.
Main Results:
- 18.5% of patients had elevated baseline PTH; one case of hypoparathyroidism identified.
- In heart failure patients, PTH and blood calcium did not significantly change during exercise stress.
- Patients with lowest ejection fraction showed decreased PTH during maximal stress.
Conclusions:
- Abnormal baseline PTH secretion is prevalent in advanced congestive heart failure.
- PTH release is blunted under stress in severe heart failure, indicating potential parathyroid dysfunction.
