Background:

Observation:

  • A 52-year-old man developed a groin mass and leg edema 4 years post-THR.
  • Imaging revealed massive cystic iliopsoas bursa enlargement compressing iliac vessels.
  • Bursography confirmed the cystic lesion but showed no communication with the hip joint.

Findings:

  • The cystic enlargement was attributed to polyethylene wear from the uncemented THR.
  • Despite no joint communication or loosening, the bursa enlarged significantly.
