Background:

  • Spontaneous mediastinal emphysema (SME) is an uncommon condition.
  • Its precise etiology remains unclear but is often associated with sudden increases in alveolar pressure.

Observation:

  • A retrospective analysis of 114 patients with mediastinal emphysema was conducted between 1980 and 2001.
  • 22 patients (19.3%) presented with spontaneous mediastinal emphysema (SME) without a clear preceding event.
  • Common symptoms included chest pain, dyspnea, and subcutaneous emphysema.

Findings:

  • All patients diagnosed with SME recovered fully.
  • No complications or recurrences were observed during the follow-up period of at least one year.
  • Diagnostic procedures like contrast esophagogram and interventions such as pleural drains were employed selectively.

Implications:

  • Spontaneous mediastinal emphysema is generally a benign condition.
  • Expectant management with a 24-hour observation period is recommended.
  • Recurrence of SME is rare, supporting a conservative treatment approach.
