Background:

  • Circumscribed acral hypokeratosis is a rare chronic disorder of cornification.
  • It predominantly affects women and presents as solitary lesions.

Observation:

  • Lesions are resistant to local conservative treatments.
  • Clinically, they appear as sharply circumscribed reddish macules on acral skin.

Findings:

  • Histologic hallmark is a circumscribed loss of the entire stratum corneum.
  • This is best visualized at the lesion border against the broad perilesional stratum corneum.

Implications:

  • Biopsy is crucial to differentiate from other non-healing acral lesions and tumors.
  • Understanding the distinct histologic features aids in accurate diagnosis.
