    Background:

    • Asthma management often involves beta 2-sympathomimetics and anticholinergic agents.
    • The efficacy of these medications varies across different asthma phenotypes.

    Purpose of the Study:

    • To compare the effects of a beta 2-sympathomimetic (salbutamol/fenoterol) and SCH 1000, alone and in combination, in asthma patients.
    • To investigate differential responses based on asthma type (atopic, non-atopic, psychogenic).

    Main Methods:

    • Double-blind, random crossover design involving 33 asthma patients.
    • Inhalation of salbutamol/fenoterol and SCH 1000, with and without placebo.
    • Measurement of Forced Expiratory Volume in 1 second (FEV1) changes over time.

    Main Results:

    • Combined sympathomimetic and SCH 1000 improved FEV1 significantly more than SCH 1000 alone for three hours (p<0.05).
    • No significant difference in SCH 1000 response was observed between atopic and non-atopic groups.
    • Patients with psychogenic asthma showed a significantly better FEV1 response to SCH 1000 from four to seven hours post-inhalation (p<0.05).

    Conclusions:

    • SCH 1000 combined with a sympathomimetic offers enhanced bronchodilation.
    • SCH 1000 demonstrates particular therapeutic promise for managing asthma with a psychogenic component.
