Background:

  • Sparganosis is a rare parasitic infection caused by the plerocercoid larvae of tapeworms in the genus Spirometra.
  • Eyelid involvement is uncommon, presenting diagnostic challenges due to migratory and painful swelling.
  • Medical treatments are often ineffective, necessitating surgical intervention.

Observation:

  • A 67-year-old male presented with a migratory, painful eyelid swelling unresponsive to medication.
  • Imaging revealed a thick-walled lesion on CT and hypoechoic tubular/cystic structures on ultrasound biomicroscopy.
  • Surgical exploration identified a 7 cm plerocercoid larva within the orbicularis oculi muscle.

Findings:

  • Histopathology confirmed a sparganum larva with a foreign body granulomatous reaction.
  • Enzyme-linked immunosorbent assay (ELISA) confirmed the diagnosis serologically.
  • Complete resolution of the lesion was achieved postoperatively.

Implications:

  • Sparganosis should be considered in the differential diagnosis of unusual or persistent eyelid masses.
  • Prompt surgical removal is crucial for effective treatment and resolution.
  • This case underscores the importance of considering parasitic infections in ophthalmology, even in non-endemic areas.
