Acquiring laparoscopic skill proficiency: does orientation matter?

Summary

Side-on laparoscopic surgery orientation initially causes more disorientation than head-on. However, with practice, skill mastery is achieved regardless of orientation, indicating adaptability in surgical training.

Area of Science:

  • Surgical Education
  • Minimally Invasive Surgery
  • Medical Training

Background:

  • Laparoscopic surgery requires specific spatial orientation skills.
  • The impact of operating orientation on learning curves is not fully understood.

Purpose of the Study:

  • To compare the time to skill mastery between side-on and head-on laparoscopic operating orientations.
  • To analyze the learning curves associated with different laparoscopic orientations.

Main Methods:

  • 131 medical students were randomized into head-on or side-on orientation groups.
  • Participants completed 10 attempts at 5 validated laparoscopic skills stations.
  • Repeated measures ANOVA and regression models analyzed time-to-completion data.

Related Experiment Videos

Main Results:

  • Head-on orientation showed faster initial completion times across all stations.
  • Side-on orientation learning curves were steeper, indicating a faster learning rate after initial difficulty.
  • Skill mastery was achieved by the tenth attempt in both groups.

Conclusions:

  • Side-on orientation presents greater initial disorientation compared to head-on.
  • Sufficient practice allows for overcoming initial disorientation and achieving skill mastery.
  • Training programs should consider orientation effects on early learning curves.
