This study found that International Headache Society (IHS) diagnostic criteria for optic neuritis (ON) may not fully meet clinical needs. Further research is required to validate these criteria for accurate ON diagnosis.

Background:

Optic neuritis (ON) is inflammatory optic neuropathy.
Diagnosis is primarily by ophthalmologists, less by neurologists.
Existing diagnostic criteria are in various classification systems.

Purpose of the Study:

To assess the clinical application of IHS optic neuritis diagnostic criteria.
To evaluate the role of ocular pain characteristics in diagnosis.

Main Methods:

Partially retrospective (140 cases) and prospective (43 cases) study.
Analysis of clinical characteristics of patients diagnosed with ON.
Focus on retro-orbital pain features and exacerbation with eye movement.

Main Results:

Retro-orbital pain, provoked or spontaneous and worsened by eye movements, was observed in a high percentage of patients.
The current IHS (2004) criteria were found to be insufficient for comprehensive ON diagnosis in clinical practice.

Conclusions:

The IHS diagnostic criteria for optic neuritis require further validation in real-world clinical settings.
Refinement of diagnostic criteria may be necessary to improve accuracy and clinical utility.
