Splenic infarction is a rare complication of malaria, even during effective treatment. Clinicians should consider this possibility in patients experiencing left hypochondrial pain during malaria therapy.

Area of Science:

  • Tropical Medicine
  • Infectious Diseases
  • Hematology

Background:

  • Malaria is a significant cause of fever in travelers returning from tropical regions, with Plasmodium falciparum being the predominant species.
  • Approximately 7000 malaria cases are reported annually in France, highlighting the importance of vigilance in returning travelers.

Observation:

  • A case of splenic infarction during effective antimalarial treatment for uncomplicated acute malaria is presented.
  • Seven additional cases of splenic infarction complicating malaria treatment were identified in the literature.

Findings:

  • Splenic infarction can occur despite appropriate antimalarial prophylaxis and treatment.
  • Unlike previously reported cases, splenic infarction occurred early in the described patient, who was older than the other seven patients.

Related Experiment Videos

  • Clinical outcomes for all reported cases, including the presented one, were favorable.
  • Implications:

    • Clinicians should be aware of splenic infarction as a rare complication of malaria treatment.
    • Left hypochondrial pain during malaria treatment warrants consideration for splenic infarction.
    • There are currently no known predictive signs for this rare complication.
