Summary

Zidovudine can cause acute hepatitis in patients with Acquired Immunodeficiency Syndrome (AIDS). An alternative drug, 2'3' dideoxyinosine, was well-tolerated, suggesting a need for clinical awareness of this rare complication.

Area of Science:

  • Hepatology
  • Virology
  • Clinical Medicine

Background:

  • Acquired Immunodeficiency Syndrome (AIDS) management involves antiretroviral therapy.
  • Zidovudine is a nucleoside reverse transcriptase inhibitor (NRTI) commonly used in HIV treatment.
  • Drug-induced liver injury is a potential complication of antiretroviral medications.

Observation:

  • A 38-year-old patient with AIDS developed acute hepatitis.
  • The hepatitis was attributed to zidovudine therapy.
  • The underlying mechanism of zidovudine-induced hepatitis remains unknown.

Findings:

  • The patient successfully transitioned to 2'3' dideoxyinosine (ddI), another NRTI.
  • ddI was tolerated well, indicating it as a viable alternative.
  • This case highlights a rare but significant adverse effect of zidovudine.

Implications:

  • Clinicians treating patients with AIDS must monitor for potential zidovudine-induced hepatotoxicity.
  • Awareness of this complication is crucial for patient safety and effective treatment modification.
  • Alternative NRTIs like ddI may be suitable for patients experiencing zidovudine-related liver issues.
