Summary
Early antibiotic therapy for acute acquired syphilitic uveitis significantly improves visual outcomes. This treatment aids in the anatomical repair of infection, even when uveitis is the initial symptom of latent syphilis.
Area of Science:
- Ophthalmology
- Infectious Diseases
- Immunology
Background:
- Syphilis can present with ocular manifestations, including uveitis, often in its latent stage.
- Acute acquired syphilitic uveitis can be the primary clinical sign of infection.
Observation:
- A retrospective case-control study analyzed 3 patients (4 eyes) with acute syphilitic uveitis.
- Syphilis diagnosis was confirmed via serological tests for treponemal antibodies.
- Two patients underwent pars plana vitrectomy for diagnostic and therapeutic purposes.
Findings:
- Specific treponemal antibodies were detected in vitreous samples from 2 eyes.
- All 4 eyes achieved a final visual acuity of 6/12 or better.
- One eye recovered to 6/6 visual acuity.
Implications:
- Prompt antibiotic treatment, primarily with penicillin, is crucial for managing ocular syphilis.
- Early intervention improves both anatomical healing and visual prognosis.
- Uveitis and neuroretinitis can be the sole presenting symptoms of latent syphilis.
