Summary

Sporadic hemiplegic migraine (SHM) presents with motor weakness during aura, distinct from familial hemiplegic migraine (FHM). SHM is now classified separately, with rare FHM gene mutations and potential Verapamil treatment.

Area of Science:

  • Neurology
  • Genetics
  • Headache Medicine

Background:

  • Sporadic hemiplegic migraine (SHM) is characterized by migraine attacks with motor weakness (hemiparesis) during the aura phase.
  • Unlike familial hemiplegic migraine (FHM), SHM occurs in individuals with no family history of identical attacks.
  • Recent studies have clarified the distinct clinical profile and classification of SHM.

Purpose of the Study:

  • To review recent scientific findings on Sporadic Hemiplegic Migraine (SHM).
  • To differentiate SHM from other migraine types, including Familial Hemiplegic Migraine (FHM) and Migraine with Typical Aura (MA).
  • To inform diagnostic criteria and potential treatment strategies for SHM.

Main Methods:

  • Review of recent scientific studies on SHM.

Related Experiment Videos

  • Analysis of clinical symptoms, prevalence, genetic mutations, and treatment responses in SHM patients.
  • Comparison of SHM patient data with FHM and typical MA populations.
  • Main Results:

    • SHM prevalence is estimated at 0.005%.
    • SHM patients exhibit clinical symptoms similar to FHM and distinct from typical MA.
    • SHM patients have a higher risk of typical MA but not migraine without aura (MO); FHM gene CACNA1A mutations are rare in SHM.
    • Verapamil shows potential as a treatment for some SHM attacks.

    Conclusions:

    • SHM is clinically distinct from typical MA and FHM, warranting separate classification.
    • The International Classification of Headache Disorders 2nd edition now separates SHM.
    • All cases of motor weakness during migraine aura should be classified as either FHM or SHM.
