Summary

An open bladder neck at rest is common in women with lower urinary tract symptoms but not diagnostic of stress incontinence. This finding suggests imaging the bladder neck at rest has questionable diagnostic value.

Area of Science:

  • Urology
  • Female Pelvic Medicine
  • Diagnostic Imaging

Background:

  • Lower urinary tract symptoms (LUTS) are common in women.
  • Urodynamic studies are crucial for diagnosing LUTS.
  • The significance of an open bladder neck at rest requires further clarification.

Purpose of the Study:

  • To investigate the association between an open bladder neck at rest and specific urodynamic diagnoses in women with LUTS.
  • To evaluate the diagnostic value of imaging the bladder neck at rest.

Main Methods:

  • Retrospective study of 2,593 women with LUTS undergoing urodynamic assessment.
  • Videocystourethrography was used to assess bladder neck appearance at rest.
  • Diagnoses included urodynamic stress incontinence, detrusor overactivity, sensory urgency, and normal urodynamics.

Related Experiment Videos

Main Results:

  • An open bladder neck at rest was observed in 29.9% of women.
  • 70% of women with an open bladder neck had urodynamic stress incontinence.
  • Women with detrusor overactivity or sensory urgency predominantly had a closed bladder neck at rest.

Conclusions:

  • An open bladder neck at rest is associated with urodynamic stress incontinence but is not diagnostic of urethral sphincter incompetence.
  • An open bladder neck at rest is not associated with urgency symptoms.
  • Imaging the bladder neck at rest has questionable diagnostic value for LUTS in women.
