Summary

Fluoroquinolones, like levofloxacin, can cause severe hypoglycemia, especially in elderly patients with diabetes on oral medications. This rare but serious side effect requires clinical awareness and careful patient monitoring.

Background:

  • Fluoroquinolones are broad-spectrum antibiotics widely used for various infections.
  • While generally safe, fluoroquinolones have been associated with rare adverse effects, including hypoglycemia.

Observation:

  • Hypoglycemia linked to fluoroquinolones is more common in elderly patients with type 2 diabetes mellitus receiving oral hypoglycemic agents.
  • A case of fatal hypoglycemia following levofloxacin administration in an elderly diabetic patient is presented.

Findings:

  • Levofloxacin, a fluoroquinolone antibiotic, can induce profound and prolonged hypoglycemia.
  • The mechanism may involve blockage of ATP-sensitive potassium channels in pancreatic beta-cells.

Implications:

  • Clinicians must be aware of the risk of severe hypoglycemia with levofloxacin in diabetic patients.
