Adults who stutter persistently (PDS) show atypical brain anatomy, particularly in white matter (WM) volumes within speech-related regions. This study reveals differences in brain asymmetry between stutterers and non-stutterers, impacting speech fluency.

Background:

  • Persistent developmental stuttering (PDS) has debated neurophysiological and neuroanatomical underpinnings.
  • Atypical anatomical asymmetries in speech-relevant brain regions are proposed to affect speech fluency in individuals who stutter.
  • This study aimed to investigate anomalous anatomy in cortical speech-language areas in adults with PDS.

Purpose of the Study:

  • To determine if adults with PDS exhibit anomalous anatomy in cortical speech-language areas.
  • To compare brain anatomy, specifically white matter (WM) and grey matter (GM) volumes, between adults with PDS and controls.
  • To investigate potential differences in brain asymmetry related to PDS.

Main Methods:

  • High-resolution magnetic resonance imaging (MRI) was used to scan adults with PDS (n=10) and matched controls (n=10).

  • An advanced voxel-based morphometry technique (augmented VBM) was employed to compare WM and GM differences.
  • Participants were matched for age, sex, hand preference, and education.
  • Main Results:

    • Increased WM volumes were observed in a right-hemispheric network including the superior temporal gyrus, inferior frontal gyrus, precentral gyrus, and anterior middle frontal gyrus in adults with PDS.
    • Non-stutterers displayed leftward WM asymmetry in the auditory cortex.
    • Stutterers exhibited symmetric WM volumes in the auditory cortex.

    Conclusions:

    • Adults with PDS demonstrate anomalous anatomy in perisylvian speech/language areas, as well as prefrontal and sensorimotor regions.
    • The findings provide strong evidence for atypical WM asymmetry in adults with PDS.
    • The causal relationship between atypical WM asymmetry and stuttering remains an open question.
