Background:

  • Pelviureteric junction obstruction (UPJO) is typically caused by crossing renal vessels.
  • Extrahepatic portal vein obstruction (EHPVO) is a rare condition affecting the portal venous system.

Observation:

  • A 26-year-old woman presented with incidentally detected EHPVO.
  • This patient also had UPJO, a condition not previously associated with EHPVO.

Findings:

  • The patient's UPJO was found to be secondary to portosystemic collaterals, not crossing renal vessels.
  • This represents a novel etiology for UPJO, linking it to portal hypertension complications.

Implications:

  • This case expands the differential diagnosis for UPJO.
