Background:

  • Primary (psychophysiological) insomnia is a common sleep disorder.
  • The efficacy of nefazodone in treating insomnia requires further investigation.

Purpose of the Study:

  • To investigate the effects of nefazodone on sleep in patients with primary insomnia.
  • To assess both objective (polysomnography) and subjective sleep parameters.

Main Methods:

  • 32 patients with primary insomnia received nefazodone (100-400 mg/night) for 4 weeks.
  • Sleep was assessed using polysomnography and subjective questionnaires (PSQI).
  • Dose titration was based on efficacy and tolerability.

Main Results:

  • 20 patients completed the study; 12 dropped out due to side effects or lack of efficacy.
  • Objective sleep showed increased sleep onset latency, decreased stage 1 and slow-wave sleep, and increased stage 2 and REM sleep.
  • Subjective sleep improved, with reduced PSQI scores, except for sleep latency.

Conclusions:

  • The high dropout rate and mixed objective/subjective results suggest the nefazodone dose range may be too high for this population.
  • Lower doses in double-blind, placebo-controlled studies are needed to evaluate nefazodone's role in treating primary insomnia.
