Summary

Vertebral arteriovenous fistulas (AVFs) are often asymptomatic or present with tinnitus. Endovascular therapy, particularly embolization, effectively treats these fistulas with high success and minimal complications.

Background:

  • Vertebral arteriovenous fistulas (AVFs) are rare vascular malformations.
  • Clinical presentation varies from asymptomatic to neurological deficits.

Purpose of the Study:

  • To review the clinical and angiographic features of vertebral AVFs.
  • To evaluate the effectiveness of endovascular treatment for vertebral AVFs.

Main Methods:

  • Retrospective review of 46 vertebral AVFs in 45 patients over 12 years.
  • Analysis of clinical presentation, etiology, location, and treatment outcomes.
  • Endovascular embolization using latex balloons was the primary treatment.

Main Results:

  • 14 patients (30%) were asymptomatic.
  • Tinnitus was the most common symptom (21 patients).
  • 41% of AVFs were trauma-related, 59% spontaneous.
  • Endovascular therapy achieved 91% complete occlusion with minimal morbidity.

Conclusions:

  • Endovascular therapy is a safe and effective treatment for vertebral AVFs.
  • It offers a minimally invasive alternative to surgery.
  • Vertebral AVF management should prioritize endovascular approaches.
