Summary

Excessive heat accumulation during prolonged exercise can be life-threatening, causing severe organ damage. Athletes must be prepared for extreme conditions to prevent heatstroke and its serious health consequences.

Background:

  • Heat accumulation in the body poses life-threatening risks, potentially damaging fluid electrolyte balance, kidneys, liver, and hematologic function.
  • Athletes engaging in prolonged strenuous activities in hot, humid climates are at increased risk of heat-related illnesses.

Observation:

  • A previously healthy 32-year-old Thai laborer collapsed during a 30 km tricycle race in 35°C heat and high humidity.
  • Post-collapse, the patient presented with convulsions, anuria, anemia, thrombocytopenia, coagulopathy, and liver impairment.

Findings:

  • Laboratory analyses revealed marked azotemia (BUN 96 mg%, Cr 10.6 mg%), elevated muscle enzymes (CPK >1000 U/L, SGOT >650 U/L), liver failure (SGPT >650 U/L), and disseminated coagulopathy.
  • The patient required intensive treatment including hemodialysis, blood-exchange transfusions, and mechanical ventilation.
