Background:

  • Severe carotid artery stenosis or occlusion increases ischemic stroke risk.
  • Reduced cerebrovascular reserve capacity (CVR) may indicate imminent ischemic events.
  • CVR assessment via vasodilative stimulus response is crucial, but its distribution in healthy individuals is poorly understood.

Purpose of the Study:

  • To investigate the distribution of cerebrovascular reactivity in healthy individuals.
  • To assess the feasibility of using dynamic T2* MRI for CVR evaluation.

Main Methods:

  • 13 healthy volunteers underwent dynamic T2* MRI.
  • Scans were acquired at alternating levels of inspiratory carbon dioxide (pCO2).
  • Cerebrovascular reactivity was measured by signal change correlated with end-tidal pCO2.

  • A high percentage of signal change was observed in grey matter (5.9 ± 1.2%), correlating linearly with pCO2.
  • White matter showed a lower signal change (1.9 ± 0.5%).
  • Hemispherically symmetrical and homogeneous distribution of CVR was found in grey matter, with mean time lags of 15 ± 4 s (grey matter) and 180 ± 12 s (white matter).

Conclusions:

  • Dynamic T2* MRI can map cerebrovascular reactivity in healthy individuals.
  • Findings suggest this method may be useful for detecting impaired cerebrovascular autoregulation in patients with compromised cerebral vasculature.
  • This technique holds potential for identifying patients at risk of ischemic stroke due to carotid artery disease.
