Summary

Granulomatous Pneumocystis pneumonia (PCP) is a rare condition that can be misdiagnosed. Early diagnosis of this severe lung infection may be improved with newer methods like PCR testing.

Background:

  • Granulomatous Pneumocystis pneumonia (PCP) is an uncommon presentation of PCP.

Observation:

  • A patient with chronic lymphatic leukemia presented with acute respiratory failure.
  • Standard diagnostic methods, including bronchoalveolar lavage (BAL), were negative.
  • Diagnosis was confirmed post-mortem via open lung biopsy.

Findings:

  • Granulomatous PCP can present atypically and evade standard diagnostic tests.
  • Bronchoalveolar lavage (BAL) may yield false-negative results in granulomatous PCP.

Implications:

  • Clinicians should consider granulomatous PCP in atypical presentations, even with negative BAL.
