Area of Science:

  • Public Health
  • Infectious Diseases
  • Epidemiology

Background:

  • Tuberculosis (TB) remains a significant global health challenge, with approximately one-third of the world's population infected.
  • In 2003, Canada reported 1,379 cases of new, active, and relapsed TB, highlighting its continued presence.

Observation:

  • Mycobacterium tuberculosis causes TB, but only 10% of infected individuals develop active disease.
  • Pulmonary TB is contagious via aerosolized droplets from coughing, talking, or singing, presenting symptoms like prolonged cough, chest pain, fever, and weight loss.
  • Extrapulmonary TB affects various sites and is typically not spread person-to-person.

Findings:

  • Key risk factors for TB infection and disease include close contact with active cases, HIV/AIDS, inadequate treatment, low income, homelessness, substance use, and healthcare occupations.
  • High-risk settings for TB transmission include correctional facilities, shelters, and long-term care facilities, as well as travel to endemic areas.

Implications:

  • Nurses play a vital role in advocating for prompt diagnosis and isolation of suspected TB cases.
  • Implementing measures such as negative pressure isolation, respirator use, and contact limitation is essential for effective nursing care.
  • Public health departments have legislated reporting requirements for positive TB tests, underscoring the importance of inter-agency collaboration.
