A 9-year-old boy with itchy skin lesions showed a rare finding of columnar epidermal necrosis, a feature previously seen only in a specific type of graft-versus-host reaction.

Background:

  • Lichen planus is a common inflammatory condition affecting skin and mucous membranes.
  • Lichenoid graft-versus-host reaction (LGHVR) is a recognized variant, often associated with transfusions.

Observation:

  • A pediatric case presented with pruritic papules and plaques on the knees.
  • Histopathological examination revealed focal columnar epidermal necrosis alongside classic lichen planus features.

Findings:

  • Columnar epidermal necrosis has been rarely reported, exclusively in transfusion-associated LGHVR.
  • This is the first reported case of lichen planus exhibiting columnar epidermal necrosis.

Implications:

  • The presence of columnar epidermal necrosis in lichen planus suggests it may be a characteristic feature of this condition.
