Acute hypothyroidism in thyroid cancer patients causes left ventricular systolic dysfunction, likely due to altered pre/afterload, not impaired contractility. Cardiac mass increased, while fractional shortening and ejection fraction may be unreliable indicators.

Purpose of the Study:

  • To evaluate the impact of acute hypothyroidism on cardiac function in athyreotic patients.
  • To assess changes in left ventricular systolic and diastolic function, cardiac mass, and vascular resistance.

Main Methods:

  • Doppler echocardiography was performed on 20 athyreotic patients during L-thyroxine therapy (euthyroid) and 5 weeks after withdrawal (hypothyroid).
  • Key cardiac parameters including left ventricular mass, volumes, systolic and diastolic function indices, and vascular resistance were measured.

Main Results:

  • Significant increases in left ventricular mass and systemic vascular resistance were observed.
  • Systolic dysfunction was indicated by changes in PEP/LVET ratio and aortic flow velocities, although fractional shortening and ejection fraction remained unchanged.
  • Diastolic function showed a decrease in E-wave velocity, but other diastolic parameters were not significantly altered. Stroke index and cardiac index significantly decreased.

Conclusions:

  • Acute hypothyroidism induces left ventricular systolic dysfunction, primarily attributed to pre- and afterload alterations rather than intrinsic myocardial contractility issues.
  • Diastolic function remained largely unaffected, while increased cardiac mass may be linked to early interstitial myxedema.
  • Fractional shortening and ejection fraction may not be reliable markers for assessing left ventricular systolic dysfunction in acute hypothyroidism.
