Magnetic Resonance Imaging (MRI) can identify aggressive intracranial meningiomas. Aggressive tumors show heterogeneous signals, more male patients, and significant peritumoral edema compared to benign types.

Background:

  • Intracranial meningiomas are common primary tumors.
  • Distinguishing aggressive from benign meningiomas is crucial for treatment planning.

Purpose of the Study:

  • To establish Magnetic Resonance Imaging (MRI) morphological criteria for identifying aggressive intracranial meningiomas.
  • To differentiate aggressive meningiomas from benign counterparts using MRI features.

Main Methods:

  • MRI features of 20 aggressive/malignant meningiomas were compared to 26 benign meningiomas.
  • Analysis included T1- and T2-weighted sequences and Gadolinium enhancement.
  • Histological data (mitoses, necrosis, invasion) were correlated with MRI findings.

Main Results:

  • Aggressive meningiomas showed a higher proportion of male patients (9/20 vs 2/26).
  • Heterogeneous signals on T1/T2 sequences due to necrosis were more frequent in aggressive tumors (P<0.01).
  • Massive peritumoral edema was significantly more frequent in aggressive meningiomas (P<0.01).

Conclusions:

  • MRI can provide morphological criteria to suspect aggressive intracranial meningiomas.
  • Tumor site and histological type had limited predictive value.
  • MRI findings like heterogeneous signals and peritumoral edema are key indicators of aggressiveness.
