Background:

  • Paroxysmal nocturnal hemoglobinuria (PNH) is a rare clonal hematopoietic stem cell disorder.
  • PNH presents with hemolytic anemia, intravascular hemolysis, hemoglobinuria, and thrombosis.
  • Stem cell transplantation is a treatment option for severe PNH cases.

Observation:

  • A 24-year-old woman with severe PNH and recurrent hemolysis underwent allogeneic peripheral blood stem cell transplantation.
  • The patient experienced frequent episodes of massive intravascular hemolysis prior to transplant.

Findings:

  • Successful engraftment was achieved 900 days post-transplantation.
  • The patient remains alive with no signs of chronic graft-versus-host disease (GVHD) or recurrent infections.
  • This represents the first successful PNH stem cell transplant in the country.
