Summary

Pre-pubertal bulimia nervosa is rare, with few documented independent cases. This study identified six patients with pre-menarchal binge eating, but only three had a potential pre-menarchal onset of bulimia nervosa.

Background:

  • Anorexia nervosa in pre-pubertal children is documented.
  • Pre-pubertal bulimic behaviors are considered uncommon, especially independent of anorexia nervosa.
  • Limited research exists on the independent occurrence of bulimia nervosa before puberty.

Observation:

  • Six patients with bulimia nervosa reported pre-menarchal binge eating without anorexia nervosa or obesity.
  • These patients were identified from a cohort of 323 individuals attending an eating disorders clinic.
  • The onset of bulimia nervosa was pre-menarchal in three patients, but they were not confirmed as pre-pubertal.

Findings:

  • Independent pre-pubertal bulimia nervosa is exceptionally rare.
  • Pre-menarchal binge eating can occur in the absence of anorexia nervosa.
  • The study highlights diagnostic challenges in identifying true pre-pubertal onset.

Implications:

  • Further research is needed to understand the etiology and presentation of early-onset eating disorders.
  • Clinical awareness should be raised for the possibility of bulimia nervosa in pre-pubertal individuals presenting with binge eating.
  • Early identification and intervention strategies may be crucial for this under-researched population.
