Area of Science:

  • Neurology
  • Infectious Diseases
  • Neurosurgery

Background:

  • Tuberculosis (TB) can affect the central nervous system (CNS), but intramedullary spinal cord involvement is exceptionally rare.
  • This study focuses on a series of rare intramedullary tuberculosis cases, emphasizing their unique clinical presentation.

Observation:

  • Five cases of intramedullary tuberculosis were analyzed, affecting four women and one man (mean age 43).
  • Two patients had a previous history of tuberculosis.
  • Clinical presentation consistently involved spinal cord compression, confirmed by spinal MRI which also revealed tuberculomas, with two patients presenting with double tumors.

Findings:

  • Complete surgical removal of the intramedullary tuberculoma was achieved in only one of the five patients.

Related Experiment Videos

  • Four out of five patients experienced a stationary outcome following treatment, indicating limited therapeutic success in this cohort.
  • Implications:

    • Intramedullary tuberculosis is a rare but serious condition requiring high clinical suspicion for timely diagnosis.
    • The limited success of surgical intervention and the often stationary outcomes underscore the need for improved therapeutic strategies for CNS tuberculosis.
    • Further research into effective treatment modalities for spinal cord tuberculomas is warranted to improve patient prognosis.
