A thalamic cyst caused intention tremor in a child. Surgical cysto-peritoneal shunting provided a long-term, tremor-free outcome after initial tapping failed.

Background:

  • Intention tremor in children can be a symptom of neurological conditions.
  • Thalamic cysts are rare intracranial lesions that can cause significant neurological deficits.

Observation:

  • A 6-year-old boy presented with intention tremor.
  • CT and MR imaging revealed a large thalamic cyst.

Findings:

  • Stereotactic aspiration of the thalamic cyst provided temporary relief from tremor.
  • Recurrence of the cyst after 18 months necessitated a cysto-peritoneal shunt.
  • The shunt procedure resulted in sustained tremor resolution for 3 years.

Implications:

  • Cysto-peritoneal shunting is an effective long-term treatment for thalamic cysts causing tremor.
