Area of Science:

  • Oncology
  • Radiotherapy
  • Breast Cancer Treatment

Background:

  • Standard postoperative radiotherapy after breast-conserving surgery involves whole breast irradiation with a tumor bed boost.
  • Local recurrences often occur near the original tumor site, prompting investigation into alternative irradiation techniques.

Purpose of the Study:

  • To evaluate partial breast irradiation (PBI) as a potential therapeutic alternative to whole breast irradiation.
  • To review existing literature on PBI techniques, including brachytherapy and intraoperative radiotherapy (IORT).

Main Methods:

  • A systematic review of literature on partial breast irradiation (PBI) published up to November 2004.
  • Inclusion of studies of any design for comparative analysis.

Related Experiment Videos

Main Results:

  • Nine multi-catheter brachytherapy studies, one balloon technique (MammoSite) study, and eight IORT trials were identified.
  • Preliminary results indicate comparable local tumor control, disease-free survival, and overall survival rates between PBI and whole breast irradiation.
  • Minor postoperative complications were reported, but the evidence base for IORT is limited.

Conclusions:

  • Phase I-II trials suggest PBI may be an appropriate alternative for select breast cancer patients.
  • Further data from ongoing phase III trials are necessary to establish PBI as a standard treatment option.
  • Whole breast irradiation remains the standard, and PBI is considered investigational.
