    Background:

    • Exercise electrocardiography (ECG) is a common diagnostic tool.
    • Exercise-induced ST elevation in patients without prior myocardial infarction (MI) warrants further investigation regarding its correlation with coronary artery disease (CAD).

    Purpose of the Study:

    • To investigate the correlation between exercise-induced ST elevation and the presence and severity of CAD.
    • To determine if specific ECG lead patterns of ST elevation correlate with particular coronary artery obstructions.

    Main Methods:

    • Retrospective review of 6040 exercise tests.
    • Correlation of ST elevation with left ventriculography and coronary angiography in 46 patients.
    • Analysis of ECG lead patterns and corresponding coronary anatomy.

    Main Results:

    • 40 of 46 patients with ST elevation had significant CAD (>=70% narrowing).
    • Anterior ST elevation (V1-V3) was associated with left anterior descending (LAD) obstruction in 86% of cases, often proximal.
    • No significant correlation found between lateral or inferior-posterior ST elevation and specific coronary artery disease.

    Conclusions:

    • Exercise-induced ST elevation is a strong indicator of CAD in patients without prior MI.
    • Resting wall motion abnormalities do not correlate with exercise-induced ST elevation.
    • Anterior exercise-induced ST elevation is a valuable predictor of significant proximal LAD obstruction.
