Summary

Stereotactic radiosurgery effectively controls brain hemangioblastomas for up to 10 years, with high tumor control rates. Regular imaging is recommended due to common recurrence outside the treated area.

Background:

  • Hemangioblastomas are rare tumors often associated with von Hippel-Lindau disease.
  • Stereotactic radiosurgery (SRS) is a treatment option for these tumors.

Purpose of the Study:

  • To review a 14-year experience with SRS for hemangioblastomas.
  • To define the role and strategy for radiosurgery in managing hemangioblastomas.

Main Methods:

  • Retrospective study of 38 hemangioblastomas in 13 patients (7 with von Hippel-Lindau disease).
  • Tumors received 18 or 20 Gy SRS.
  • Median clinical follow-up was 36 months; median radiological follow-up was 35 months.

Main Results:

  • 97.4% tumor control rate at 10 years.

  • 80.8% actuarial survival rate at 10 years.
  • 38.5% of patients developed new or growing lesions outside the irradiated area; larger tumors (>0.5 cm³) showed more peritumoral enhancement.
  • Conclusions:

    • Gamma Knife surgery is safe and effective for hemangioblastoma control up to 10 years.
    • Higher doses and smaller tumors correlate with better outcomes.
    • Regular follow-up and early re-treatment are recommended for new or growing lesions.
