New methicillin-resistant Staphylococcus aureus (MRSA) colonization frequently occurs after liver transplants. Urinary catheter use, bleeding, and fluoroquinolone use increase risk. Postoperative screening is recommended.

Background:
- Methicillin-resistant Staphylococcus aureus (MRSA) is a significant cause of infection post-orthotopic liver transplantation (OLT).
- While MRSA colonization is a known risk factor for infection, its incidence and predictors after OLT remain unclear.
Purpose of the Study:
- To determine the incidence of MRSA colonization after OLT.
- To identify factors associated with MRSA acquisition in OLT recipients.

- Prospective cohort study of 60 OLT patients (2000-2002).
- Nasal swab surveillance cultures at baseline and at weeks 2, 6, 13, and 26 post-transplant.
- Exclusion of patients with baseline MRSA colonization.

- 15% (9/60) of patients acquired MRSA colonization during the median 72-day follow-up.
- Factors associated with increased colonization risk included urinary catheter use (>5 days), postoperative bleeding, and preoperative fluoroquinolone use.
- No patients without these risk factors developed MRSA colonization.
Conclusions:
- Nasal MRSA carriage is commonly acquired following OLT.
- Postoperative screening for MRSA carriage is crucial for reducing nosocomial transmission in OLT patients.
- Further research is needed to develop predictive models for targeted screening of high-risk OLT recipients.
