Nocturnal frontal lobe epilepsy involves motor attacks during sleep. Ictal SPECT revealed altered brain activity in specific regions during these episodes.

  • Characterized by paroxysmal motor attacks during sleep, it can manifest as brief paroxysmal arousals (PAs), nocturnal dystonia, and wandering.

Observation:

  • The study details a patient with lifelong NFLE experiencing complex nocturnal events.
  • Ictal SPECT was performed during a paroxysmal arousal (PA) episode.

Findings:

  • Ictal SPECT demonstrated hyperperfusion in the right anterior cingulate gyrus and cerebellar cortex.
  • Hypoperfusion was observed in the right temporal and frontal associative cortices during the PA.

Implications:

  • These findings suggest specific neurobiological underpinnings for NFLE paroxysmal arousals.
