Summary

The prevalence of end-stage renal disease (ESRD) has nearly doubled, with increasing numbers of older patients requiring dialysis. Kidney transplantation offers excellent survival, but donor shortages and malignancy risks are concerns.

Area of Science:

  • Nephrology
  • Public Health
  • Epidemiology

Background:

  • The Swedish Registry for Active Treatment of Uraemia (SRAU) was established in 1991 to document demographic data for end-stage renal disease (ESRD) patients.
  • ESRD management involves dialysis and kidney transplantation, with evolving patient demographics and treatment outcomes.

Purpose of the Study:

  • To describe the prevalence, incidence, comorbidity risk factors, and survival rates of ESRD patients undergoing dialysis and/or kidney transplantation in Sweden.
  • To analyze trends in ESRD between 1991 and 2002.

Main Methods:

  • Data collected from all 65 dialysis and transplant units in Sweden, reported to the SRAU.
  • Longitudinal follow-up of patients until death, documenting demographic data, diagnoses, treatments, and outcomes.

Related Experiment Videos

Main Results:

  • ESRD prevalence increased by 75% (to ~750 PMP) over 12 years, with a stable incidence (~125 PMP) since 1997.
  • New patients are older (majority >65 years), with diabetic nephropathy and nephrosclerosis as leading causes.
  • Five-year survival: 23.1% on dialysis, 85.5% post-transplantation. Cardiovascular disease is the primary cause of death; malignancy risk increases post-transplantation.

Conclusions:

  • ESRD prevalence has nearly doubled, with a rise in older patients and those with comorbidities.
  • Kidney transplantation survival is excellent, but donor shortages and increased malignancy post-transplant are significant concerns.
