Summary

Hyperbaric oxygen (HBO) therapy for carbon monoxide (CO) poisoning does not show a clear benefit in preventing neurological sequelae based on current randomized trials. More research is needed to determine HBO's role in CO poisoning treatment.

Area of Science:

  • Emergency Medicine
  • Toxicology
  • Hyperbaric Medicine

Background:

  • Carbon monoxide (CO) poisoning is a significant global health concern.
  • Previous studies suggested hyperbaric oxygen (HBO) may prevent neurological issues.
  • HBO is widely used for CO poisoning despite limited high-quality evidence.

Purpose of the Study:

  • To evaluate the effectiveness of HBO versus normobaric oxygen (NBO) in preventing neurological sequelae after acute CO poisoning.
  • To analyze data from randomized controlled trials (RCTs).

Main Methods:

  • Searched MEDLINE, EMBASE, and Cochrane Trials Register.
  • Included RCTs of non-pregnant adults with acute CO poisoning.
  • Two reviewers extracted data on participants, interventions, and outcomes.

Related Experiment Videos

Main Results:

  • Six RCTs of varying quality were analyzed.
  • Four trials found no benefit of HBO; two suggested a benefit.
  • Pooled analysis showed no significant benefit (OR 0.78, 95%CI 0.54-1.12), with considerable heterogeneity and methodological flaws.

Conclusions:

  • Current RCTs do not confirm HBO's efficacy in reducing neurological sequelae in CO poisoning.
  • Further high-quality, multi-center RCTs are necessary to clarify HBO's role.
  • The optimal treatment strategy for CO poisoning requires additional investigation.
