Endurance exercise training significantly improved symptoms for most patients with orthostatic intolerance. This study suggests that exercise is a valuable treatment for this condition.

Background:

  • Orthostatic intolerance (OI) is a syndrome of chronic symptoms like light-headedness and fatigue upon standing.
  • It is often associated with orthostatic tachycardia and elevated norepinephrine levels.

Purpose of the Study:

  • To evaluate the protective effect of endurance exercise training on orthostatic symptoms.
  • To determine the efficacy of exercise training as a treatment for orthostatic intolerance.

Main Methods:

  • 2768 military recruits were screened for OI; 36 were diagnosed via tilt-table testing.
  • 31 participants with OI were randomized into a 3-month jogging intervention group or a control group.
  • Symptom scores and tilt-table test results were assessed before and after the intervention.

  • After training, only 6 of 16 participants in the exercise group still had OI, compared to 10 of 11 in the control group (P=0.008).
  • The exercise group showed a significant improvement in average symptom scores (1.79 to 1.04), while the control group had no significant change (2.09 to 2.14).

  • Endurance exercise training significantly improves symptoms in the majority of patients with orthostatic intolerance.
  • Endurance training should be considered as a treatment option for individuals suffering from orthostatic intolerance.
