Summary

Dialysis is a viable treatment for elderly patients with chronic renal failure. While early mortality is high, some octogenarians experience effective treatment and survival comparable to younger patients.

Background:

  • Chronic renal failure (CRF) incidence increases with age.
  • Age is no longer a contraindication for renal replacement therapy.

Purpose of the Study:

  • Characterize Scottish dialysis patients aged 80+ at first treatment.
  • Compare outcomes of elderly dialysis patients with other serious illnesses.

Main Methods:

  • Retrospective survey of elderly dialysis patients in Scotland (1994-2001).
  • Comparison with lung cancer and myocardial infarction patients of similar age.

Main Results:

  • 5.4% of new dialysis patients were aged 80+.
  • 90% received hemodialysis; CRF of unknown cause was most common diagnosis.
  • Elderly patients had poorer physical health but similar social/mental health.
  • Median survival for elderly dialysis patients was 459 days, compared to 141 days for lung cancer and 1242 days for MI patients.

Conclusions:

  • Dialysis can be effective for octogenarians with end-stage renal failure.
  • Treatment offers a potential benefit for a subset of elderly patients.
