Blood volume in angina pectoris.

H Aström, A Holmgren, B Jonsson

    British Heart Journal
    |April 1, 1979
    PubMed
    Summary

Patients with severe angina pectoris often have significantly lower blood volume than predicted, particularly those without signs of heart failure. This finding is crucial for understanding preoperative conditions in coronary bypass surgery candidates.

    Area of Science:

    • Cardiovascular Medicine
    • Surgical Pre-operative Assessment

    Background:

    • Disabling angina pectoris affects numerous male patients requiring coronary bypass surgery.
    • Preoperative assessment is critical for optimizing patient outcomes in cardiac surgery.

    Purpose of the Study:

    • To investigate blood volume status in male patients with disabling angina pectoris prior to coronary bypass surgery.
    • To determine the prevalence of reduced blood volume and its potential correlation with clinical factors.

    Main Methods:

    • Blood volume was measured in 60 consecutive male patients.
    • Patients were undergoing preoperative investigation for coronary bypass surgery.
    • Data on medication (diuretics, digitalis, beta-blockers) and signs of myocardial insufficiency were recorded.

    Main Results:

    • A significant reduction in blood volume was observed in 58 out of 60 patients (21% lower than predicted, P < 0.001).
    • No correlation was found between blood volume deviation and the use of diuretics, digitalis, or beta-blockers.
    • The most pronounced decrease in blood volume was noted in patients without signs of myocardial insufficiency.

    Conclusions:

    • Male patients with disabling angina pectoris frequently exhibit a reduced blood volume before coronary bypass surgery.
    • Reduced blood volume is more prominent in patients without overt signs of myocardial insufficiency.
    • These findings highlight the importance of assessing blood volume in this patient cohort for surgical planning.
