Background:

  • Gastrointestinal mesenchymal tumors (GIMTs) encompass a spectrum of neoplasms originating from the gastrointestinal tract's mesenchymal stem cells.
  • Gastric schwannoma is an exceptionally rare GIMT, accounting for only 0.2% of gastric tumors and 4% of benign gastric neoplasms.

Observation:

  • A 24-year-old female presented with upper gastrointestinal bleeding.
  • Endoscopic evaluation revealed a bleeding, ulcerated, round submucosal tumor in the gastric body.

Findings:

  • Surgical resection and subsequent pathological examination identified a spindle cell tumor.
  • Immunohistochemical analysis showed strong S-100 protein positivity and negativity for CD34, CD117, actin, HHF-35, desmin, melan-A, and HMB-45, confirming a diagnosis of gastric schwannoma.

Implications:

  • This case underscores the importance of considering rare diagnoses like gastric schwannoma in patients with gastrointestinal bleeding.
  • Accurate pathological and immunohistochemical evaluation is crucial for diagnosing gastric schwannoma and differentiating it from other mesenchymal tumors.
