Summary

Electrophysiologic study effectively identifies causes of unexplained syncope, especially in heart disease patients. Treating identified abnormalities reduces syncope recurrence but does not impact mortality.

Background:

  • Unexplained syncope is a common clinical problem.
  • Electrophysiologic study (EPS) is a diagnostic tool for syncope evaluation.

Purpose of the Study:

  • To assess the diagnostic yield of EPS in unexplained syncope.
  • To compare abnormality detection rates in patients with and without heart disease.
  • To evaluate the impact of treating EPS-detected abnormalities on syncope recurrence and mortality.

Main Methods:

  • EPS was performed in 111 consecutive syncope patients.
  • Patients were categorized based on the presence or absence of heart disease.
  • Follow-up assessed syncope recurrence and mortality in treated versus untreated patients.

  • Abnormalities were found in 42% of patients with heart disease vs. 16% without (P<0.01).
  • Treatment of abnormalities reduced syncope recurrence (5% vs. 24%, P<0.05) and improved 2-year syncope-free survival (0.94 vs. 0.72, P<0.05).
  • EPS had no mortality and low morbidity (8%).

Conclusions:

  • EPS is valuable for diagnosing syncope, particularly in patients with heart disease.
  • Treatment guided by EPS findings reduces syncope recurrence.
  • EPS is safe and effective for unexplained syncope evaluation.
