Summary

A chronic cough in a 56-year-old man was diagnosed as Bronchial Associated Lymphoid Tissue (BALT) lymphoma. This rare pulmonary lymphoma presented as infiltrates mimicking pneumonia, highlighting the need for definitive diagnosis in non-infectious cases.

Background:

Chronic cough can be a challenging symptom with diverse etiologies.
Pulmonary infiltrates often suggest infectious processes like pneumonia.
Early and accurate diagnosis is crucial for effective management of lung conditions.

Observation:

A 56-year-old male presented with a 6-month history of chronic cough and bronchial sounds.
Chest X-ray and CT scan revealed lingular and bilateral infiltrates with air bronchograms.
Previous imaging from seven years prior showed similar lingular infiltrates.

Findings:

Bronchoscopic transbronchial biopsies confirmed the diagnosis of Bronchial Associated Lymphoid Tissue (BALT) lymphoma.
The slow-progressing, disseminated nature of the lymphoma and the patient's good performance status influenced treatment decisions.
