Genetic variations in the interleukin-6 (IL-6) gene are associated with intervertebral disc disease (IDD)-related sciatica. Specific IL-6 single nucleotide polymorphisms and haplotypes show increased frequency in patients, suggesting a role in discogenic pain.

Area of Science:

  • Genetics
  • Immunology
  • Orthopedics

Background:

  • Intervertebral disc disease (IDD) causing sciatica affects approximately 5% of the population.
  • While environmental factors contribute, IDD has a significant genetic component.
  • Inflammation is increasingly recognized as a key factor in IDD etiology.

Purpose of the Study:

  • To investigate the role of inflammatory mediator genes in IDD-related sciatica.
  • To analyze genetic variations in candidate cytokine genes, focusing on IL-6.

Main Methods:

  • Genotyping of 16 polymorphisms in 10 cytokine genes (including IL1A, IL1B, IL6, TNFA) in 155 IDD patients and 179 controls.
  • Mutation analysis of IL1A, IL1B, IL6, and TNFA genes in a subset of patients.
  • Statistical analysis of allele and genotype frequencies, including haplotype analysis for IL-6 SNPs.

Related Experiment Videos

Main Results:

  • No disease-causing mutations were found in IL1A, IL1B, IL6, or TNFA.
  • Significant differences in allele frequencies were observed for an IL-6 single nucleotide polymorphism (SNP), T15A in exon 5 (P=0.007).
  • Specific IL-6 genotypes (AA, AT) and the GGGA haplotype were more prevalent in patients with IDD-related sciatica, indicating a strong association with the disease.

Conclusions:

  • Genetic variations within the IL-6 gene are significantly associated with intervertebral disc disease-related sciatica.
  • The findings support the role of IL-6 genetic variations in the development of discogenic pain.
  • Further research into IL-6's inflammatory pathways may offer new therapeutic targets for IDD.
