Summary

General practitioners often overlook the systolic pressure index (SPI) in peripheral arterial disease (PAD) management, despite its diagnostic importance. While exercise advice and anti-platelet use are common, smoking cessation support remains insufficient.

Area of Science:

  • Vascular Medicine
  • Epidemiology
  • General Practice

Context:

  • Peripheral Arterial Disease (PAD) management guidelines recommend specific diagnostic and therapeutic practices.
  • The ATTEST study provides real-world data on PAD care in metropolitan France.
  • General practitioners (GPs) are central to the diagnosis and management of PAD.

Purpose:

  • To evaluate current general medicine management practices for peripheral arterial disease (PAD).
  • To assess adherence to recommended guidelines for PAD care, including diagnostic tests and risk factor management.
  • To identify areas for improvement in PAD management by general practitioners.

Summary:

  • Analysis of 6227 patients in the ATTEST study revealed suboptimal use of the systolic pressure index (SPI) for PAD diagnosis (17%).

Related Experiment Videos

  • While investigation of other atherothrombotic sites (50%), advice on physical activity (~66%), and anti-platelet prescription (92.2%) were frequent, smoking cessation interventions were notably insufficient (7%).
  • Significant gaps exist in the systematic application of recommended PAD diagnostic and risk factor management strategies by general practitioners.
  • Impact:

    • Highlights the critical need for enhanced GP education and adherence to PAD management guidelines.
    • Underscores the importance of improving smoking cessation support within PAD patient care.
    • Suggests a need for closer collaboration between GPs and vascular specialists to optimize patient outcomes.
