Summary

This case study investigates factitious lithiasis, where a patient presented with recurrent "stones" of non-urinary origin. Advanced analysis revealed these were exogenous materials, not true urinary calculi.

Background:

  • Recurrent lithiasis can present diagnostic challenges, especially when conventional tests are negative.
  • Factitious lithiasis, or self-induced urolithiasis, is a rare condition requiring specific diagnostic approaches.

Observation:

  • A 19-year-old woman reported spontaneous expulsion of small

  • despite negative radiologic and laboratory findings.
  • Preliminary morphological analysis suggested a non-metabolic origin for the expelled samples.
  • The diverse composition of the

Findings:

  • stones
  • as exogenous materials, including pure calcite and mixed calcite-silicates.
  • No correlation was found between the patient's crystalluria and the composition of the expelled samples.
  • This confirmed the diagnosis of factitious lithiasis, accounting for 0.1% of analyzed urinary calculi in Western Algeria.
  • Implications:

    • Highlights the importance of considering factitious causes in recurrent lithiasis cases with atypical presentations.
    • Demonstrates the utility of advanced physical analysis techniques in diagnosing non-metabolic urinary conditions.
    • Guides clinicians toward specific complementary investigations for suspected factitious lithiasis.
