Summary

Vitrectomy for epiretinal membrane improves vision in patients with good preoperative acuity. However, cataract surgery is crucial for sustained visual gains in phakic (natural lens) eyes.

Background:

  • Epiretinal membranes (ERMs) can impair vision.
  • Vitrectomy is a surgical option for ERMs.

Purpose of the Study:

  • To assess visual outcomes of vitrectomy for ERMs in eyes with pre-existing good visual acuity (20/50 or better).

Main Methods:

  • Retrospective analysis of 40 consecutive cases of vitrectomy for ERMs.
  • Single surgeon series evaluating visual acuity and complications.

Main Results:

  • Mean visual acuity improved from 20/50+2 to 20/40+2 post-vitrectomy (P=.02).
  • Pseudophakic eyes showed significant improvement (20/32-2) compared to phakic eyes (20/50-2).
  • Visual acuity improvement of ≥2 lines occurred in 42.4% of pseudophakic eyes vs. 14% of phakic eyes.

Conclusions:

  • Vitrectomy offers visual benefits for ERMs in eyes with good initial acuity.
  • Cataract surgery is essential for achieving long-term visual acuity enhancement in phakic eyes undergoing vitrectomy.
