Background:

Observation:

  • A 22-year-old male presented with symptoms consistent with mumps encephalomyelitis.
  • Magnetic resonance imaging revealed extensive T2 signal hyperintensities in the spinal cord (C4-C6 and T5-T10).
  • This represented the most significant spinal cord involvement reported in adult mumps cases.

Findings:

  • The patient received supportive therapy, oral steroids, and early rehabilitation.
  • Neurological examination at six months showed bilateral 2/5 paresis in the lower extremities.

Implications:

  • Mumps encephalomyelitis, though uncommon in adults, can lead to severe neurological deficits.
  • Early diagnosis, comprehensive treatment including rehabilitation, and literature review are crucial for managing this condition.
